Participation Terms & Consent (the “Terms”)
By registering for, accessing, or using the all.health application (“the App”), you acknowledge and agree to the following Terms:
Enrollment in Hines Chronic Condition Management Program
By consenting to the use of the App, you expressly consent to being enrolled in the Hines Chronic Condition Management program. This program provides services designed to support individuals with chronic health conditions through care coordination and ongoing management.
Consent to Communications
You consent to receive communications related to your enrollment and participation in the program. These communications may be provided via phone calls, voicemail, text messages, emails, or through the App itself. Such communications may include, but are not limited to, appointment reminders, health management information, program updates, and messages from Hines Chronic Condition Managers.
Your Responsibilities
You agree to provide accurate and up-to-date information as requested by the App or program staff. You are responsible for notifying Hines Chronic Condition Managers if your contact information changes to ensure continuity of communication.
No Guarantee of Medical Outcomes
Participation in the program and use of the App does not replace the professional medical judgment of your healthcare provider. The program is intended as a support tool, and no guarantee of specific health outcomes is made. Always consult your physician or healthcare provider with questions regarding your medical condition or treatment.
Privacy and Data Use
Your personal information will be collected, used, and stored in accordance with our Privacy Policy and applicable law. By using the App, you consent to such collection and use.
Sharing of Health Information
By using the App, you acknowledge and consent that your Protected Health Information (PHI) may be collected and shared between the App and Hines Chronic Condition Management. This exchange of information is necessary to provide care coordination, health monitoring, and program services. Your PHI will be safeguarded in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable laws.
Changes to These Terms
We may update or modify these Terms at any time without prior notice. Any changes will be effective immediately upon posting within the App or on our website. Your continued use of the App following the posting of revised Terms constitutes your acceptance of those changes.
